On the left side of this page are several forms. Print out the Enrollment Form, AOB Form and Doctor Order Form. Complete the Enrollment Form and AOB Form then call us at 1-800-866-4997 and one of our Customer Service Representatives will go over the information with you.
OPTION #2
On the left side of this page are several forms. Print out the Enrollment Form, AOB Form and Doctor Order Form. Complete the Enrollment Form and AOB Form then mail all 3 forms to us at Hock's Pharmacy, 535 S. Dixie Drive, Vandalia, Ohio 45377. Once we receive the forms, one of our Customer Service Representatives will contact you to review the information with you.

Complete the Enrollment Form and AOB Form then mail the Enrollment Form and AOB Form to us at Hock's Pharmacy, 535 S. Dixie Drive, Vandalia, Ohio 45377. Take the Doctor Order Form to your physician and have them fill it out completely and mail it to us at Hock's Pharmacy, 535 S. Dixie Drive, Vandalia Ohio 45377 or they can Fax it to us at 937-898-9340. Once we receive all the forms, one of our Customer Service Representatives will contact you to review the information with you.
